Joshua Rein, DO, FASN, is a nephrologist, physiologist, and hypertension specialist. He is an Assistant Professor of Medicine in the Barbara T. Murphy Division of Nephrology at the Icahn School of Medicine at Mount Sinai and a Staff Physician Scientist at the James J. Peters Veterans Affairs Medical Center. He received his medical degree from the New York College of Osteopathic Medicine, completed internal medicine at Mount Sinai Beth Israel where he excelled in clinical training having received the Dr. Ira B. Cohen Award in Recognition for the Most Outstanding Medical House Officer, and completed clinical and research nephrology fellowships at Mount Sinai Hospital.
Dr. Rein's clinical interests include hypertension, acid/base, and electrolyte disorders. Dr. Rein's research, funded by a Veterans Affairs Career Development Award, utilizes preclinical models to discover new mechanisms underlying the renal regulation of fluid and electrolyte homeostasis. His long-term goal is to translate key observations from preclinical studies into an enhanced understanding of human disease and therapeutics.
Dr. Rein teaches hypertension, acid/base, and electrolyte physiology to nephrology fellows, residents, and medical students. He publishes original research and contributes original hypotheses and knowledge to the medical community through invited and voluntary presentations and publications. He also publishes unique clinical observations of acid/base and electrolyte disorders as case reports and case series.

Research
Hydrodynamic forces, including fluid shear stress, circumferential stretch and drag/torque on apical cilia, regulate ion transport in the cortical collecting duct (CCD) of the kidney. Studies aimed at examining pathways and regulation of transepithelial transport in this nephron segment are largely limited to experiments in 2D cell culture monolayers, while renal tubular epithelia grown in their native 3D geometry provide a more physiologically relevant environment. We are developing and characterizing a novel in vitro 3D microphysiological kidney-on-a-chip model that recapitulates the in vivo geometry of the CCD that promises to provide a relevant model for studying tubular ion transport physiology and addressing a variety of clinically important questions.
