Nurse Practitioner in the Brookdale Department of Geriatrics and Palliative Medicine
Sarah Romanelli provides comprehensive primary care for geriatric patients aged 75 and older, delivering in-person and telehealth urgent visits, pre-op exams, post-hospitalization transitions, Medicare annual wellness visits, and chronic disease management. She also leads quality improvement projects focusing on population health outcomes, including Patient-Centered Medical Home quality metrics, falls assessment and prevention, emergency department utilization, and safe opioid prescribing. Sarah manages telephone triage, responds to patient portal messages, and coordinates same-day care through in-basket management.
Her recent work includes serving as an adjunct instructor and advisor at New York University’s Rory Meyers College of Nursing since May 2021, where she facilitates lab components for advanced physical and health assessment courses and advises graduate nursing students. In October 2023, she worked as an Interprofessional Education Facilitator at Johns Hopkins University School of Nursing, leading discussions on the opioid epidemic with students across various health disciplines. From July 2022 to June 2023, Sarah was a Junior Coach at Mount Sinai Hospital’s Brookdale Department of Geriatrics and Palliative Care, teaching quality improvement methodology and overseeing project progression. Before her career as a nurse practitioner, she was an emergency department nurse for six years.
Sarah is currently pursuing a Doctor of Nursing Practice at Johns Hopkins University, with an expected graduation in May 2025. She earned a Master of Science in Adult-Gerontology Primary Care Nurse Practitioner with an Advanced Certificate in Palliative Care from New York University in September 2019. She also holds a Master of Public Health from New York University, obtained in May 2019, and a Bachelor of Science in Nursing from Boston College, completed in May 2014.
Her publications include a textbook chapter in "Advanced Practice Palliative Nursing" and articles in the Journal of Gerontological Nursing and Journal of the American Association of Nurse Practitioners. She has also written an editorial for Cognoscenti.
Sarah has received several awards, including the STAR Award and the Ambulatory Care Geriatrics Practice Employee Recognition Award from Mount Sinai Hospital, as well as the Ambulatory Nursing Excellence “Quality Improvement Excellence” Award and the Advanced Practice Nurse Excellence “Rookie of the Year” Award.
Complementing her professional achievements, Sarah's strong interest in global health is evident from her work in the Dominican Republic, Cuba, Nepal, Uganda, and Argentina. Her certification in hospice and palliative care further enriches her practice, as she enjoys incorporating this expertiseinto her work.
