Dr. Stephanie Chow is an Assistant Professor in the Brookdale Department of Geriatrics and Palliative Medicine at Mount Sinai Hospital. She serves as Director of the Aging, Life, Interventions, Goals & Needs (ALIGN) Program, an innovative “high risk” ambulatory care model that delivers comprehensive multidisciplinary care to older frail patients living with serious illness and complex care needs. ALIGN specializes in co-managing several vulnerable patient groups, including patients deliberating surgery, requiring repeated emergency room evaluations, and transitioning between discharge settings. Dr. Chow also serves as co-champion of Diversity, Equity and Inclusion for her department and is a passionate advocate for promoting equity in the underserved patient and inter-professional health system communities.
