
Theresa A Soriano, MD
About Me
Dr. Theresa Soriano is a Professor in the Departments of Medicine, Geriatrics and Palliative Medicine at ISSMS. She currently serves as Senior Vice President for Care Transitions and Population Health at Mount Sinai St. Luke's, with previous roles as Deputy CMO for Population Health in Mount Sinai Health Partners, Acting Chief of the Division of General Internal Medicine, and Director of the Mount Sinai Visiting Doctors Program, the nation’s largest academic home-based primary and palliative care program. Dr. Soriano received her MD and Masters in Public Health at Mount Sinai School of Medicine and completed her residency in Internal Medicine at University of Miami – Jackson Memorial Hospital in Miami, FL. She joined the faculty of Mount Sinai’s Division of General Medicine in 2004.
Dr. Soriano’s career interests include care models for underserved and/or complex populations, clinical quality, interdisciplinary care teams, workforce development and advocacy in home-based primary and palliative care. She has presented and been recognized for her work in home-based primary and palliative care and in community-oriented care models at the regional and national levels, and has published in major medical and healthcare professional journals.
